Commuters can see water dripping inside Delhi Metro coaches from air conditioning vents plus the air conditioning vents are NOT clean.
There are no case studies on transmission of air borne diseases inside metro trains. One research article on this topic is Evidence for airborne infectious disease transmission in public ground transport - a literature review by O Mohr, M Askar, S Schink, T Eckmanns, G Krause, G Poggensee published in Eurosurveillance, Volume 17, Issue 35, 30 August 2012. Another article titled The Effect of Metro Rail on Air Pollution in Delhi by Deepti Goel and Sonam Gupta brings out another facet of Metro trains in respect of non-commuters too.
